The Digital Waste Profile Form below is a universal pre-requisite for all materials to be reviewed for approval by WRI prior to acceptance of a new waste stream. Additional information and supporting documentation may be required for approval, depending on a review of the Waste Profile submission by WRI personnel.
The Digital PCW Certification Form is a mandatory requirement by FDEP necessary to validate the classification of a non-hazardous waste stream as Petroleum Contact Water (PCW). This form is only necessary when requesting approval for treatment of PCW.
Please complete, print and sign all required forms prior to submitting to WRI for review. WRI accepts profiles by fax and/or email.
